Acute Tick Bite Telemedicine Visit ($375)

For Wisconsin residents with an acute tick bite, recent in onset, who want a clinical evaluation from a Lyme-literate, ILADS-member provider. This visit type is exclusively for an acute tick bite. It is not for chronic Lyme, long-term Lyme symptoms, or other tick-borne illness. If you are looking for treatment for any of those, please use our regular new patient pathway so we can route you through the right channel.

What is included

• Secure online intake form (about 5 to 10 minutes to fill out)
• Initial 60 minute video visit with Jamie Lorrigan, DNP, APNP, a Lyme-literate, ILADS-member Adult-Gerontology Acute Care Nurse Practitioner
• Lyme-literate early treatment plan. Jamie may recommend antibiotics, targeted supplements, herbal antimicrobials, supportive care, or a combination, with the course length matched to your situation.
• Any prescription routed to your preferred pharmacy. Any recommended supplements or supports come with a clear list.
• SOAP note documentation in our secure HIPAA-compliant system
• Up to two follow-up video visits at 4 weeks and 8 weeks after the initial visit, scheduled and paid separately at $225 each (about 30 to 45 minutes)
• Care under the acute bite pathway runs up to 12 weeks total

Eligibility

• Currently a Wisconsin resident or physically located in Wisconsin for the visit
• 13 years of age or older
• Not enrolled in or eligible for Medicare, Medicaid, or CHIP
• Acute tick bite, recent in onset, not a chronic or ongoing condition

Visit format

All visits are conducted by video only. You will need a device with video capability and a stable internet connection.

Important

The intake form alone is not a clinical decision. Jamie Lorrigan will speak with you on the video visit to evaluate before any treatment is initiated.

All intake forms and consents must be completed before your appointment begins. If they are not completed in time, your appointment will be rescheduled once. If they are still not completed by the rescheduled appointment, the visit will not proceed and you forfeit the visit fee.

Cancellation and no-show policy. Cancellation more than 24 hours before your appointment is refunded at 50 percent. Cancellation within 24 hours is non-refundable and a $50 late-cancellation fee applies. No-show without notice is non-refundable and a $50 no-show fee applies.

Eligibility-hold refund. If we find after payment that you do not meet the eligibility for this pathway (Wisconsin location, age 13 or older, not on Medicare or Medicaid, acute and not chronic), you may request a 50 percent refund of the visit fee.

This visit type does not establish you as a regular patient of Green Medicine of Wisconsin. After your care episode, you may opt to become an established patient through our regular new patient pathway. Care under the acute bite pathway ends 12 weeks after your initial visit.
